Q311 · Category G: Quality management and compliance systems (Q301–Q350)
A deviation is any unforeseen event that departs from an approved instruction or established standard, such as an SOP. A mature QMS must have a clear deviation-handling procedure. The process begins when any employee who identifies a deviation reports it immediately. The deviation must then be documented in detail and initially assessed to determine its potential impact on product quality. Depending on the level of impact, immediate control measures, such as quarantining the relevant products, may be necessary. The key step is root cause analysis to establish why the deviation occurred. Based on the analysis, implement corrections/corrective action to address the current problem and preventive action (part of CAPA) to prevent similar issues from recurring. The entire process, from identification through to closure, must be fully documented.

📘 Regulatory basis

The Good Distribution Practice (GDP) for Medicinal Products requires all events that deviate from established procedures to be recorded and investigated. Any significant deviation that may affect the quality of medicines must be thoroughly investigated, and effective corrective and preventive action must be taken, with all actions documented.

📗 Drew Tang's recommendations

We encourage clients to establish a “blame-free” reporting culture that encourages employees to report deviations proactively rather than hide them. Treat deviations as opportunities to improve the system. When conducting root cause analysis, do not stop at “human error”; investigate whether there are systemic causes such as poorly designed processes, inadequate training or insufficient resources. An effective deviation management system is evidence that the QMS can improve itself.

📕 Risk notice

Systematically ignoring deviations, or dealing superficially with deviation investigations and corrective actions, is a serious compliance deficiency. During an inspection, the regulatory authority will use deviation records to assess the health of your quality system. A high number of recurring deviations without effective preventive measures indicates that your QMS is out of control and may lead to stringent regulatory action.
