Retail pharmacy, wholesale distribution and manufacturing of human medicinal products in Czechia do not operate under a single licence. A pharmacy is a pharmaceutical-care service within healthcare services. The regional authority for the location of the pharmacy grants the healthcare-services authorisation, subject to a favourable binding opinion from SÚKL on the premises’ technical and material equipment. Wholesale distribution and manufacturing (including the scope covering imports from third countries) are licensed and inspected by the State Institute for Drug Control (SÚKL). A wholesale distributor or manufacturer does not automatically acquire another authorisation by obtaining company-formation documents, a pharmacy authorisation or a product registration. This guide was prepared by Rengangyongsheng (Hong Kong) Limited and is explained by Tang Shangyong (Drew Tang) in his capacity as a business manager; he is neither a pharmacist nor a regulatory official and makes no guarantee of approval or commitment to a fixed or expedited timeline.
This article covers pharmaceutical business licensing matters based on local public sources. Separate requirements may apply to retail, wholesale, manufacturing, product registration, imports and exports, and local rules; cross-check against the competent authorities’ current requirements before proceeding with a project.
Quick comparison of 3 types of business activity
Distinguish licensing pathways according to the activities actually undertaken. For personnel, premises, product and cross-border requirements, consult the corresponding sections and official sources below.
| Business activity | Scope of application | Competent authority | Key personnel |
|---|---|---|---|
| Pharmacy / pharmaceutical-care authorisation | Pharmaceutical-care services provided to the public by a pharmacy; these should be addressed separately from general pharmaceutical wholesale, product registration and imports from third countries. | The regional authority for the pharmacy’s location decides on the healthcare-services authorisation; SÚKL first issues a favourable binding opinion on the premises’ technical and material equipment. | Pharmaceutical care must be provided by personnel with the relevant health-profession qualifications. Applicable requirements for the authorisation and professional representative must be checked under the Healthcare Services Act, implementing rules and the specific case; this guide does not presume a pharmacist coverage ratio or on-duty hours. |
| Wholesale distribution authorisation for human medicinal products | Distribution activities including the procurement, storage and supply of medicinal products, supply within the EU and export to third countries; dispensing/supplying medicines to patients by a pharmacy is not included in this definition of distribution. | SÚKL。 | The application must identify qualified personnel for distribution and include documents such as their qualifications, CV, job duties, contract and criminal-record certificate; the same qualified person may perform this role under only one distributor’s authorisation. |
| Manufacturing authorisation for human medicinal products (including the scope covering imports from third countries) | Manufacturing activities such as full or partial manufacture and packaging/repackaging, as well as importing human medicinal products from non-EU third countries; these cannot be covered by a general wholesale authorisation. | SÚKL。 | SÚKL’s manufacturing service is authorised under the Medicines Act, the implementing rules on manufacturing and distribution, and GMP. The public service page does not fully list the qualification and experience thresholds for all qualified personnel. Applicants should check with SÚKL in light of their proposed processes, batch release and quality responsibilities. |
Scope and entities
This record is limited to pharmacy retail, wholesale distribution and manufacturing of human medicinal products in Czechia. SÚKL’s manufacturing service expressly applies to legal persons or natural persons authorised to conduct business; the wholesale application form likewise separately requests identifying information for natural persons or legal persons. The government service page for pharmacy healthcare-services authorisation is also addressed to natural persons or legal persons. The official pharmaceutical sources reviewed do not provide a uniform minimum registered capital, prescribed company form or foreign ownership ratio that can be applied directly; no such assertion is therefore made. [1] [2]
Czechia does not use a federal/state model of separate licensing for this matter: pharmacy authorisations fall under the regional authority for the pharmacy’s location, while manufacturing and wholesale authorisations fall under the nationwide SÚKL. Do not assume that an existing regional authorisation automatically covers a pharmacy opened in another region or a change of location; confirm with the authority for the relevant location. [3]
— This record does not cover veterinary medicines, medical devices, health foods or cosmetics. SÚKL has a classification role in determining product boundaries, but non-medicinal products may be regulated by other authorities. [9]
— Company registration, tax, construction, recognition of professional qualifications and general investment screening may also apply separately and are not waived by pharmaceutical authorisation.
Authorities and allocation of legal responsibilities
SÚKL is the nationwide authority for human medicinal products under the Ministry of Health. The official text of the Medicines Act lists its powers to decide on product registrations and amendments to them, manufacturing authorisations, wholesale authorisations and corresponding amendments, suspensions and revocations. In the pharmacy authorisation pathway, SÚKL issues the binding opinion on the premises’ technical and material equipment. [4]
The “authorisation to provide healthcare services” for a pharmacy is decided by the regional authority where the facility is located. This local authorisation mechanism operates alongside SÚKL’s nationwide pharmaceutical authorisations. Applicants should allow for separate documentation and review time and should not submit an application to only one of the two authorities. [3] [5]
— Cross-border movement of controlled substances: the relevant department of the Ministry of Health issues import and export permits, and customs checks the documents at declaration. [10]
— “Medicinal products” in this record means human medicinal products; veterinary medicines are handled under a separate veterinary-authority system. [9]
Pharmacies and retail
Pharmaceutical care provided in a pharmacy is a healthcare service. Before opening, apply to the regional authority for the pharmacy’s location for authorisation to provide healthcare services. The government service page states that the authorisation is granted by a decision of the regional authority and must be obtained before services begin. [3]
Before applying to the regional authority for a pharmacy authorisation, the proposed operator must apply to SÚKL for a favourable binding opinion on the technical and material equipment of the healthcare facility. SÚKL issues its opinion within 30 days of receiving a written application. The opinion is a mandatory attachment to the application to the regional authority for authorisation to provide pharmaceutical-care services, and SÚKL may require corrections following an on-site inspection. [5] [6]
— Application to the regional authority: identify the competent regional authority based on the facility address and submit the application and attachments required by that authority; there is no mandatory uniform national form. [3]
— A pharmacy authorisation is an authorisation for pharmaceutical-care services. Each individual product must still have the relevant product registration or another statutory basis for marketing; a pharmacy licence is not product registration.
Wholesale distribution
Wholesale distribution of human medicinal products requires an application to SÚKL for a distribution authorisation. The application must specify the types and scope of distribution proposed, all storage locations, the qualified person for distribution and contact details. It must include evidence of the right to use the premises, a GDP/SDP compliance questionnaire and warehouse plan, distribution procedures, the qualified person’s documentation and proof of payment. SÚKL makes its authorisation decision only after verifying the complete documentation and conducting an on-site inspection. [2]
Distribution is not merely a logistics registration. The official Medicines Act defines distribution to include procurement, storage, supply, supply within the EU and export to third countries. SÚKL requires authorisation holders to comply with GDP/SDP and may, following an inspection, issue a certificate stating whether the inspection found compliance and enter it in the European database. [4] [7]
— Before changing application details such as company information, the type/scope of distribution, storage address or qualified person, apply for an amendment to the authorisation. [2]
— Different rules may apply to activities involving only active substances or excipients; the conclusions for this medicinal-product wholesale authorisation should not automatically be applied.
Manufacturing and third-country imports
A legal person or a natural person authorised to conduct business that intends to manufacture human medicinal products in Czechia must first obtain a manufacturing authorisation from SÚKL. The authorisation also covers imports of human medicinal products from non-EU third countries. SÚKL inspects compliance with the Medicines Act at the manufacturing site. Applicants must comply with the Medicines Act, the rules on manufacturing and distribution, and the European Commission’s GMP guidelines. [1]
A manufacturing authorisation is not limited to full production. Official legislation includes partial manufacture, packaging, repackaging and packaging changes within the scope of manufacturing authorisation. Exceptions such as preparations made by a pharmacy under the statutory conditions must not be interpreted as a general exemption for commercial manufacturing. [4]
— Apply before commencing manufacturing or imports from third countries. The SÚKL service page specifies a statutory processing period of 90 days from the application, excluding time allowed for correcting the application. [1]
— Changes to the scope of manufacturing or imports require an application to amend the authorisation before implementation; a scope change may involve an on-site inspection. [8]
Professional personnel
As a healthcare service, pharmacy services must be provided by qualified healthcare professionals. The Healthcare Services Act establishes a professional-representative system: the representative must have an employment or similar relationship with the service provider (subject to statutory exceptions), perform the role to an extent sufficient to ensure professional management, and may hold this position for no more than two service providers. The specific minimum staffing requirements for a pharmacy project must still be confirmed under the applicable implementing rules and through review by the regional authority. [11]
Wholesale applications must identify a qualified person. The person’s qualifications, experience, job duties, contract and criminal-record certificate are all listed among the public application documents. The same qualified person may perform this role under only one distributor’s authorisation. The public manufacturing service page requires compliance with conditions including GMP, but does not fully publish the qualification and years-of-experience standards for all qualified manufacturing personnel; these must not be supplied by inference. [2] [12] [1]
— This article does not stipulate that a pharmacist must cover two-thirds of business hours or any fixed shift ratio; the Czech official materials reviewed do not provide such a uniform figure.
— Where foreign qualifications, appointment of a professional representative or batch-release responsibilities are involved, confirm the specific case with the relevant competent authority and qualified local professionals.
Premises and facilities
A pharmacy must first obtain SÚKL’s favourable opinion on the technical and material equipment of the healthcare facility. The application for this opinion should be submitted once the relevant conditions have been met, and the opinion must be attached to the application for authorisation to the regional authority. The official service page does not publish a uniform area figure for this procedure; therefore, this article does not specify a minimum area. [5] [6]
A wholesale warehouse must be non-residential premises that comply with general building requirements and form an operationally enclosed, functionally coherent whole. If the relevant categories are actually distributed, it must also have sufficient storage capacity, receiving and dispatch areas, and segregated areas for returned, recalled or awaiting-destruction products, as well as separate areas for controlled substances, radioactive medicines, gases or refrigerated products. [12]
— The addresses of all wholesale warehouses must be included in the licence application; evidence of the right to use the premises, such as a lease or proof of ownership, must be submitted with the application. [2]
— A pharmacy address is important location information in the healthcare services authorisation; relocation must be handled through the authorisation amendment procedure.
Documentation and procedures
The pharmacy route is generally as follows: first satisfy the facility and personnel requirements; submit a written application to SÚKL for a favourable equipment opinion; then apply to the regional authority where the facility is located for healthcare services authorisation. Regional authorities are not legally required to use a uniform national form; application materials should follow the requirements published by the competent region. [5] [3]
Wholesale applications should follow SÚKL form DIS-8 and its list of attachments. After receiving the application, SÚKL reviews it for completeness and may request additional documents in writing; a distribution authorisation decision is issued only after the statutory and implementing requirements have been met and an on-site inspection completed. [2]
Manufacturing applications are submitted to SÚKL, which conducts an inspection of the site. If deficiencies are identified in the application or during the on-site inspection, the applicant may be asked to remedy them; if they are not remedied, the procedure may be discontinued. The public service page does not list every attachment for manufacturing applications; use the current SÚKL form/guidance VYR-27 and confirm the requirements through case-specific communication. [1] [13]
— The applicant entity, scope of activities and locations should each be fixed before applying; do not begin operating with an expanded scope or at an additional site and apply afterwards.
— Retain version records of submissions, remedial filings, inspections, decisions and approved scope as a basis for subsequent amendments and supply-chain audits.

Fees, timeframes and validity periods
Pharmacies: The government service page lists a fee of 1,000 Czech koruna for a regional authority to receive an application for healthcare services authorisation, with a statutory processing period of 30 days. The fee for an application to SÚKL for a pharmacy equipment opinion is 1,000 Czech koruna; SÚKL issues the opinion within 30 days of receiving the written application. [3] [5]
Wholesale: The SÚKL application page lists an administrative fee of 2,000 Czech koruna for a distribution authorisation or its amendment. The fee for professional work involving an on-site inspection of one warehouse is 41,900 Czech koruna, with 21,600 Czech koruna for each additional warehouse. The page also states a maximum processing period of 90 days, subject to review of the materials and an on-site inspection. Fees should be checked against the current version of UST-29 at the time of submission. [2]
Manufacturing: The SÚKL service page lists 90 days for an initial authorisation, 30 days for an administrative amendment and 90 days for a change to the manufacturing/import scope. These periods exclude time during which the procedure is suspended pending remedial action. The public service page did not list manufacturing authorisation fees in this verification, nor was a general uniform validity period or renewal cycle for the three main authorisations identified. Do not infer either; confirm against the decision and the fee schedule in force at the time of submission. [1] [8]
— Statutory periods stated on websites are part of the regulatory processing framework; they are not equivalent to the actual delivery timeline for a complete project or a guarantee of approval.
— This record does not assert that there is a uniform annual fee, renewal date or automatic extension.

Operations, GMP and GDP
Services continuously provided by a pharmacy must be delivered at the locations and within the scope specified in its healthcare services authorisation, and the personnel and technical and material equipment must remain suitable for those services. The Health Services Act requires a provider to notify the competent authority in writing of changes to information that may lead to suspension, amendment, revocation or expiry of the authorisation; notification should generally be made within 15 days of becoming aware of the change. [11]
Wholesalers must maintain GDP/SDP. SÚKL states that following an inspection it issues a post-inspection certificate confirming whether the distributor complies with GDP/SDP, and enters authorisation and certificate details in the European database. Manufacturing applicants must maintain GMP compliance; changes to the manufacturing/import scope trigger an amendment review and may require an on-site inspection. [7] [1] [8]
— Wholesale: Continuously maintain qualified personnel, storage conditions, recall procedures, distribution records and the authorised scope.
— Manufacturing: Continuously ensure that the actual approved status matches the authorisation scope, and manage manufacturing and import in accordance with GMP.
— Pharmacy: Continuously meet the conditions underpinning the regional healthcare services authorisation and SÚKL equipment opinion.
Share transactions, changes and termination
Where the proposed change to a pharmacy healthcare services authorisation concerns the scope of services, the address of a service location, the professional representative named in the authorisation or the authorisation’s validity period, the provider must apply to amend the authorisation decision. Other changes that may affect the authorisation conditions should generally be notified to the competent authority in writing within 15 days of becoming known. After a change, evidence relating to the changed conditions must be submitted. [11]
Where details of the entity, distribution scope, warehouse address or qualified personnel stated in a wholesale authorisation application subsequently change, SÚKL requires an amendment to the authorisation. A manufacturer intending to change the conditions under which its authorisation was granted must also apply to SÚKL for an amendment before changing manufacturing/import activities. After manufacturing operations cease, an application may be made to cancel the manufacturing authorisation. [2] [8] [14]
— The official materials reviewed did not establish a uniform “automatic transfer” rule directly applicable to all share transfers, changes of control, mergers, asset transactions or replacements of licence holders; do not claim that a licence transfers automatically with shares or assets.
— Before a transaction, confirm each matter with the competent authority and local professional advisers, taking into account whether the licence holder, authorised locations, professional representative/qualified personnel, business scope or marketing authorisation holder will change.
Product registration, imports and controlled medicines
Medicinal products for human use must be registered before being marketed in the Czech Republic. In the registration process, SÚKL assesses the safety, efficacy and quality information, as well as information on indications, contraindications, dosage, dispensing classification, package leaflet and packaging, submitted by the future marketing authorisation holder. This product registration is separate from the pharmacy, wholesale and manufacturing authorisations for business premises. [15]
For imports of medicinal products from third countries, SÚKL expressly states that only a person holding a manufacturing authorisation covering “import from third countries” may carry them out; a general wholesaler cannot import solely on the basis of a wholesale authorisation. If the imported medicinal product is not registered in any EU Member State or across the EU, prior SÚKL consent for that import is also required under the Medicines Act. [16]
For controlled substances and preparations containing them, each import or export from a third country requires a permit issued by the Ministry of Health’s designated department and submitted with the customs declaration; the permit is subject to a specified validity period and quantity. For medicinal products included on the Ministry of Health’s shortage-risk list, distribution abroad must also be notified electronically to SÚKL using DIS-18 and, if not restricted, may be carried out within the originally notified scope only after 15 working days from notification. [10] [17]
— Check registration status, holder, packaging and available information in the product database, but a database search does not replace verification of the business operator’s authorisations or the import route. [18]
— Controlled-substance permits, consent to import unregistered medicines, manufacturing authorisation for third-country imports and general wholesale authorisation must be assessed separately according to the goods and route.
